If you are having problems with these forms, please download the latest version of Adobe Acrobat Reader. These forms must be saved to your computer and sent as an attachment in an email. In most cases, these forms will work on Mac computers. These forms are not intended for use with iPads.

Student Profile

SHP Student Profiles assess students' eligibility for awards. The SHP Student Profile should contain the students' most up-to-date contact information along with student demographics. All students are encouraged to keep their student profile sheets up-to-date following each semester. Information related to awards and honors and other accomplishments should be documented on the Student Profile.

Tutoring

Tutoring is available to all SHP students free of charge at any time during the semester. If you would like tutoring, please see the information below specific to your department. If you would like to become a tutor, please see your advisor. Tutors are only hired at the request of the individual departments, pending available job openings. Tutors are paid $13.50 per hour.

Nutrition & Metabolism

Students seeking tutoring should complete the Request for Tutoring Services (PDF) form and submit to the email address provided on the form. Tutoring will be assigned based on available tutors. If a tutor is available, they will contact you within 5 days to set up a day and time to meet. Once assigned to that tutor, you can have as many sessions of tutoring as needed for the remainder of the semester. Please note that faculty may decide to request you be assigned to a tutor for a specific course. If this occurs, your department will assign your request directly to a tutor.

Background Checks

Students Entering Spring 2018 or After

New incoming students will utilize CastleBranch. Students will receive an email at their UTMB student email providing the link to complete the requirements.

Instructions for Students Who Entered Fall 2017 or Before

Students who do not have a CastleBranch account should follow the link below. You may be asked to provide employment history through the data-collection process. The employment history investigation can be significantly shortened if a legitimate e-mail address for your employers is provided, along with a supervisor name. Also, if you were contracted through a temporary agency, please provide the contact information of that agency rather than the company you were placed with.

To proceed with your standard background check, visit the Sterling Talent Solutions - Standard Package website.  The cost of this package is $40.47

To proceed with your OIG/HCFA & GSA package, visit the Sterling Talent Solutions-OIG/HCFA & GSA Package website.  The cost of this package is $19.18

Compliance Training

UTMB and the School of Health Professions requires all students, whether full time, part time or distance students, to take mandatory compliance courses each year. You must complete the Annual Student Required Training within 30 days of enrollment and annually thereafter within 60 days of the start of the semester. Failure to complete the training modules by the required due date will result in a HOLD on your student record and/or disciplinary action.

Note: Students involved in Research will have extra mandatory courses assigned and they must be completed by the due dates, as well.

Each semester, the School of Health Professions recognizes academic excellence for students in good standing with the following GPA:

Foreign Transcripts

Effective with the Fall 2012 entry term, applicants from international institutions must use one of the credentialing services approved by UTMB to provide a verification and a course by course evaluation of transcripts in order to be able to be considered for admission to programs at UTMB.

Applicants are no longer required to send official transcripts from the issuing institution to UTMB since we are now utilizing these services.

The list of private organizations which provide foreign transcript evaluation services can be found on the UTMB Enrollment Services website. UTMB does not endorse any particular company listed.

Study Rooms

Study rooms are meant to support academic study and research ONLY. Inappropriate conduct such as excessive noise, sleeping, and/or recreational use (parties) is not permitted. The University reserves the right to remove users from rooms based on inappropriate conduct, limit future study room use, and follow up with appropriate University Campus Police and disciplinary action.

Guidelines for SHP Student Study Rooms

Classrooms serve as "Classrooms" Weekdays - Monday through Friday from 7:00 am to 6:00 pm. If rooms are not in use during these hours, students are permitted and welcomed to use as study space. However, if you are using a study room you must leave when asked by a group with a reservation.

After 6:00 pm, rooms are available for Student Study as specified below.

  • Available Weekdays (Monday through Friday) 6:00 pm - 11:30 pm
  • Available Weekends (Friday & Saturday) from 8:00 am - 11:30 pm
  • Study rooms are intended primarily for group use. Individuals can reserve a study room when they need a more private space to practice a presentation or participate in an online discussion.
  • Although the group study rooms are a more private space, they are not sound proof. Please keep the noise down so as not to disturb others.
  • Food and/or drink are allowed in specified rooms provided that the food or drink are not messy. Clean up the room before you leave so that it is ready for the next group. Don't lose this privilege.
  • Overnight use is strictly prohibited.
  • Reservations have priority. If you are using a study room without a valid reservation, you must leave when asked by a group with a valid reservation.
  • All study rooms are open and are available (subject to the policies above) on a first-come-first-served basis.
  • Study rooms may be reserved and are available during the hours outlined below. You do not need to reserve them on the weekends because they have been reserved for your use but they are first-come-first-served.

To reserve one of these conference/study rooms or a specific classroom or lobby during the day (Monday - Friday, 8:00 am - 6:00 pm), or if you have questions, please contact shp.academicaffairs@utmb.edu.

Currently, the following rooms in SHP are available on a first-come-first-served basis for all SHP students wishing to study after school hours. You do not need to reserve them on the weekends because they have been reserved for your use but they are first-come-first-served.
